Program Name: Empower PT & AT Appointment Reminders

Program Description: Automated text message reminders for physical therapy appointments

By consenting to receive SMS messages from Empower Physical Therapy & Athletic Training, PLLC, you agree to the following terms:

1. MESSAGE TYPES

You will receive the following types of SMS messages:

  • Appointment reminders (sent 48 hours before scheduled sessions)

  • Appointment confirmations (sent when sessions are scheduled)

  • Secure links to view your upcoming appointments

  • General service notifications related to your care

2. MESSAGE FREQUENCY

You can expect to receive approximately 2-4 text messages per week, with a maximum of approximately 12-15 messages per month, depending on your appointment schedule.

3. MESSAGE AND DATA RATES

Message and data rates may apply as determined by your mobile carrier. Empower PT & AT does not charge for SMS messages, but your carrier's standard messaging rates will apply.

You may opt out of receiving SMS messages at any time by:

  • You consent to receive automated text messages at the number provided
